'Transfer of Care Around Medicines (TCAM) service' - Go-live information from the Southampton launch event

Thank you to 75+ people that attended this important launch event. The buzz and enthusiasm which was generated and felt in the room was great to part of.

TCAM is now live and the first few referrals are happening as we speak. It is now important to embed the new behaviours within our teams to ensure that PharmOutcomes is regularly checked for hospital referrals and that when these occur that they are followed up within the next few days, with actions recorded onto PharmOutcomes to demonstrate activity. The key message is Read, Ring, Report.

The feedback from the evening has been very good. I do hope that this service is most helpful to all community pharmacies in directing patients most at need of a medicine review to your door, enabling you to provide both fantastic care as well as helping them to stay healthier, thus reducing potential hospital readmissions and driving down cost within NHS secondary care.

This is a perfect opportunity for community pharmacy to demonstrate its vital role in providing healthcare advice and services and enhancing its reputation further. Lets all now work together to ensure that TCAM in Southampton is a massive success.
